Client Consent Form

I acknowledge by signing this agreement that I have been given full opportunity to ask any and all questions which I might have. I specifically acknowledge I have been advised of the facts and matters set forth below and I agree as follows:

I have advised my artist if I have: diabetes, epilepsy, hepatitis, HIV-AIDS, haemophilia or any other communicable diseases, heart conditions or take medicine which thins the blood.

I am not pregnant or nursing.

I am not under the influence of alcohol or drugs.

I do not have medical skin conditions such as, but not limited to: keloid scarring, eczema, psoriasis, moles or sunburn in the area to be tattooed that may interfere with said tattoo.

I acknowledge it is not reasonably possible for the representatives and employees of Black Opal Tattoo Studio to determine whether I might have an allergic reaction to the tattoo and I agree to accept the risk that such reaction is possible.

I acknowledge that an infection is always possible as a result of obtaining a tattoo, particularly if the aftercare instructions are not followed.

I agree to follow aftercare instructions while my tattoo is healing and agree that any touch-up work due to my own negligence will be done at my own expense.

I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change my tattoo.

I acknowledge that I am over the age of 18 and have truthfully represented to my tattoo artist that the obtaining of a tattoo is by my choice alone.

I agree to the price given by my artist for said tattoo and have given full consent for application of my tattoo.

I fully understand that BLACK OPAL TATTOO STUDIO AND ARTIST DOES NOT ACT AS A MEDICAL PROFESSIONAL. Any suggestions made to me are NOT to be construed as or substituted for advice from a medical professional.

I hereby release the tattoo artist at Black Opal Tattoo Studio from all manner of liabilities, claims, actions and demands in law, or in equity.
